Okay so I figured it would be great for everyone to have this Gerber form that I use to send in to get the $5 in Gerber coupons. I cannot find this on the Gerber website anymore, but it still works as I just print it out and mail it in every time I have 10 UPCs from Gerber Graduate products. It is good until 12/31/08 or while supplies last. I have done this about 20 times so far as we are really big into the Gerber dinners and snacks. I even had to start taking the UPCs off before I put them away in the cabinet because my husband kept forgetting to take the UPC off and it killed me to think that I could have gotten a coupon from the UPC!!! So once you send in the form with your UPCs (and yes you can do it over and over again but I only do 1 envelope at a time with 10 UPCs) then Gerber will send you a mailer with $5 in coupons only for Gerber Graduate products.
